If your school, organization or civic club would like a donation from Kicks please use the following procedure:

  1. Submit the request on letterhead stationery.
  2. Describe the event with its name, date and purpose.
  3. Include the number of $5 Game Cards you are requesting.
  4. Please provide the printed name of your authorized representative with the person’s signature.
  5. Fax your letter to:

Your Kicks donation request will be reviewed. When approved, your organization will receive the Kicks $5 Game Cards. (Note: One $5 Game Card may be redeemed per person per visit. They may not be combined.)
